Patient Policies

We would like to welcome you to Dermatology Associates of Uptown. We look forward to taking care of your dermatologic needs.

Please familiarize yourself with the following policies:

After-Hours Emergencies:

• If we are not in the office, a practitioner can be reached after hours for emergencies only. Just dial our office number, (972) 283-8979, and you will be given a number to reach the practitioner on call.

Appointments:

• We ask that you give us a 24-hour notice if you need to reschedule or cancel an appointment. There is a $25.00 fee for no shows and a $75.00 fee for surgery no-shows.
• We try to run on time so please be on time to your appointment. If this is your first visit we ask that you arrive twenty (20) minutes early so we can process your paperwork and insurance.
• Payment is due at the time of service. There is a $25.00 fee for all returned checks.

Prescriptions:

• Please call your pharmacy for refills.
• Please give us 24 to 48 working hours to complete all prescription refill requests.
